1. By purchasing the product, you agree to the terms and conditions and accept the warranty information available regarding each product purchased. You agree that you will make a follow up appointment with your healthcare professional.
2. All Dexcom users must review all training materials provided with the product. If the Dexcom user requires further training on the product, they must contact their healthcare professional or contact AMSL Diabetes for further assistance.
3. If glucose alerts and readings do not match symptoms or expectations, user should use a blood glucose meter.
4. It is the Dexcom user’s responsibility to seek appropriate advice when necessary and to use the device as per usage instructions.

7. AMSL reserves its right to modify the terms and conditions or the contents of the subscription program at any time at their sole discretion. Any changes to the subscription program will be released via AMSL’s website.
8. In compliance with the Privacy Act 1988 (Cth) and AMSL's Privacy Policy, the Dexcom user’s records will be maintained in accordance with the National Privacy Principles.
9. The Dexcom user assigns indemnity to AMSL from and against all claims of whatsoever nature (to the maximum extent permitted by law) relating (whether directly or indirectly) to the Dexcom CGM Systems.
10. This product is continuous glucose monitoring system indicated for the management of (type 1, type 2, gestational) diabetes in people age 2 years and older where self-monitoring of blood glucose (SMBG) is indicated. It is not indicated for use in patients on dialysis treatment. Receivers, transmitters and sensors are not for resale. Use of non-approved platforms will invalidate the warranty for the receiver, sensor and transmitter.
